Release and Waiver of Liability

In consideration for being permitted to participate in the Thomas Elite Summer League operated by

Thomas Elite Hockey LLC, I, the undersigned, hereby acknowledge and agree to the following:

1. Assumption of Risk:

I understand that participation in ice hockey involves physical activity and inherent risks of

injury, including but not limited to collisions, falls, equipment failure, and other unforeseen

hazards. I voluntarily assume all risks, known and unknown, associated with participation in the

league.

2. Release of Liability:

I, on behalf of myself, my heirs, personal representatives, assigns, and next of kin, hereby release

and hold harmless Thomas Elite Hockey LLC, its owners, directors, coaches, employees,

volunteers, agents, sponsors, and any affiliated facilities or partners (collectively, "Releasees")

from any and all liability for injury, illness, property damage, or death resulting from my

participation in the Summer League.

3. Medical Treatment:

I authorize Thomas Elite Hockey LLC to obtain emergency medical treatment in the event of

injury or illness. I understand that I am responsible for all medical expenses incurred as a result of

my participation.

4. Rules & Conduct:

I agree to abide by all rules, including the league’s non-contact and zero-tolerance policies for

fighting or unsportsmanlike conduct. I understand that players under the age of 18 must wear a

full cage, mouth guard, and neck guard at all times.

5. Media Release:

I consent to the use of my image or likeness in promotional materials, media coverage, and social

media associated with the Thomas Elite Summer League.

6. Indemnification:

I agree to indemnify and hold harmless the Releasees against any claims arising from my

participation in the event, including attorney’s fees and court costs.
